    Dos de los acontecimientos más trascendentales del ser humano como el “nacer” y el “morir” acontecen habitualmente en el ámbito sanitario, ambos pueden considerarse procesos clínicos, transcurren en un tiempo más o menos definido en el proceso de nacer y cada vez más definido en el proceso de morir (“morscerta, hora certa”). La compleja sociedad actual carente de valores perdurables, está cuestionando los “fines medicina” en aras de la tecnocracia y la asistencia a la carta, planteando la existencia humana como valor individual, relativo a la decisión de la persona y con finitud programada, todo ello auspiciado por los avances científicos que han logrado aumentar la supervivencia humana hasta límites insospechados hasta hace poco tiempo. La moralidad y legalidad de la muerte elegida se instala en el reconocimiento liberal de la autonomía de la persona. La bioética optimalista responde al reto de los que defienden que la eutanasia, suicidio asistido y muerte digna como “actos médicos” amparados por ley en base al derecho a la libertad del ciudadano para elegir cómo y cuándo desea morir. La única respuesta que puede dar la medicina es a través de los cuidados paliativos, en todo tiempo, modo y lugar de la existencia humana.

    Two of the most transcendental events of the human being such as "being born" and "dying" usually occur in the health field, both can be considered clinical processes, take place in a more or less defined time in the process of being born, and gets especially defined defined in the process of dying (morscerta, hora certa). The complex current society lacking lasting values, is questioning the "ends medicine" for the sake of technocracy and assistance to the letter, posing human existence as individual value, relating to the decision of the person and with finitude programmed, all sponsored for the scientific advances that have managed to increase human survival to unsuspected limits until recently. The morality and legality of the chosen death is installed in the liberal recognition of the autonomy of the person. Optimistic bioethics responds to the challenge of those who defend that euthanasia, assisted suicide and dignified death as "medical acts" protected by law based on the citizen's right to freedom to choose how and when he wishes to die. The only answer that medicine can give is through palliative care, at all times, in the way and place of human existence.
