La fragilitat com a base del diagnòstic situacional davant el repte de la complexitat clínica al final de la vida

Ante el reto de la complejidad clínica -especialmente en aquellos pacientes en situación de final de vida-, en esta tesis doctoral hemos propuesto dos escenarios con el objetivo de ayudar a los profesionales y pacientes a hacer frente a la incertidumbre inherente a ésta complejidad: Por un lado, hemos puesto de manifiesto la necesidad de replantear algunos de los paradigmas del conocimiento actual, proponiendo nuevos conceptos, reordenando conocimiento ya consolidado y finalmente planteando un nuevo marco conceptual con el objetivo de operativizar éste planteamiento. Por otra parte, hemos aportado algunas nuevas evidencias, constatando que las variables dinámicas son los indicadores más consistentes para la identificación precoz de personas con pronóstico de vida limitado y necesidades de atención paliativa; también hemos diseñado y evaluado un Índice de Fragilidad basado en la Valoración Integral Geriátrica (IF-VIG) que lo avalan como una herramienta sencilla (respecto al contenido), rápida (en el tiempo de administración) y con excelente capacidad discriminativa y predictiva.
In order to be able to face the “wicked problem” of the clinical complexity –especially at the end-of-life situation– in this PhD we have developed two settings with the aim of helping professionals and patients to deal with the problem’s inherent incertitude: On the one hand, we bring to life the need to rethink some of the current knowledge’s paradigms, proposing new concepts, reordering the already consolidated knowledge and finally setting out a framework for the purpose of putting this approach into operation. On the other hand, we have tried to further explore this knowledge whilst providing some new evidence: we have verified that the dynamic variables are the most consistent indicators of the early identification of people with limited prognosis and palliative care needs. We have also designed and evaluated the IF-VIG -a Frailty Index based on the Comprehensive Geriatric Assessment (Valoració Integral Geriàtrica, in catalan)-; the analysed cohort guarantees the IF-VIG as a simple (in terms of content), quick (in terms of time), with an excellent discriminative capacity (with the situational diagnosis) and also a predictive (with high correlation to mortality) tool.
