This article analyzes the actions of the government of Puerto Rico to confront a smallpox epidemic that occurred between 1875 and 1876. It starts from the medical police concept to identify the actions of the authorities that will safeguard the life of the population. Conflicts between the island and municipal government stand out, since while the municipal government tried to implement measures to prevent the arrival of the disease, the island government stopped them so that the economy would not be affected. Among the actions of the municipality of San Juan are the efforts to eliminate the city walls, a way to end the overpopulation of the city; carry out a vaccination and revaccination campaign against smallpox, because a significant part of the population was not vaccinated; and establish a place of isolation to separate the infected poor, to avoid contagion from the rest of the population. This epidemic was significant in the history of the city of San Juan because at the end the sanitary structures were transformed.
