      Introducción: la otalgia, es una afección de las estructuras externas, medias e internas del oído que pueden o no manifestarse en torno a la articulación temporomandibular y zonas circundantes, debutando por clínica inicial o posterior a procedimientos clínicos. Presentación del caso: paciente femenina de 63 años de edad con antecedente de exéresis de ameloblastoma en rama mandibular izquierda y colocación de material de osteosíntesis hace 38 años. Presentando sintomatología de 22 años de evolución con dolor crónico de moderada a fuerte intensidad a la apertura bucal e irradia a hemicara izquierda y oído ipsilateral, exacerba 24 horas previo la consulta (24 de octubre de 2022) sin alteraciones al examen físico por nuestra área. Se solicita valoración por servicios de odontología maxilofacial y radiografía panorámica donde se evidencia ruptura de alambre de osteosíntesis. Posteriormente, es llevada a mesa quirúrgica para recambio y colocación de una placa reconstructiva N°2.7 en defecto anatómico mandibular fijado con tres tornillos en extremo inferior y dos tornillos en extremo superior y se realiza cierre de defecto quirúrgico por planos con sutura Vicryl 3-0 y Nylon 5-0. Discusión: se debe considerar las afecciones de la esfera otorrinolaringológica. Carrasco et al. (2022) expusieron que la hemimandibulectomía y colocación de placa de reconstrucción con componente condilar por medio de la reconstrucción con placas de titanio ofrece una correcta función y estética al paciente. Conclusión: el uso del material quirúrgico tipo titanio mejora los defectos anatómicos y logra resultados permanentes en el paciente.Introduction: otalgia, is a condition of the external, middle and internal structures of the ear that may or may not manifest around the temporomandibular joint and surrounding areas, debuting with initial symptoms or after clinical procedures. Case presentation: A 63-year-old female patient with a history of excision of ameloblastoma in the left mandibular ramus and placement of osteosynthesis material 38 years ago. Presenting symptoms of 22 years of evolution with chronic pain of moderate to strong intensity when opening the mouth and radiating to the left side of the face and ipsilateral ear, exacerbated 24 hours prior to the consultation (10/24/22) without alterations to the physical examination in our area. An assessment is requested by maxillofacial dentistry services and a panoramic x-ray showing a rupture of the osteosynthesis wire. Subsequently, she is taken to the surgical table for replacement and placement of a reconstructive plate No. 2.7 in the mandibular anatomical defect fixed with 3 screws at the lower end and 2 screws at the upper end and closure of the surgical defect is performed by planes with suture. Vicryl 3-0 and Nylon 5-0. Discussion: Conditions in the otorhinolaryngology field should be considered. Carrasco et al. (2022) stated the hemimandibulectomy and placement of a reconstruction plate with a condylar component, whereby the reconstruction with titanium plates offers correct function and aesthetics to the patient.
