Nitrofurantoína: eficacia y seguridad para la profilaxis de la infección del tracto urinario

    Objetivos: La nitrofurantoína ha sido utilizada para el tratamiento y la profilaxis de la infección del tracto urinario (ITU) durante más de 60 años, la aparición de nuevas resistencias de E. coli a distintos antibióticos ha hecho que en los últimos 20 años vuelva a incluirse entre los tratamientos profilácticos de elección. Esta revisión bibliográfica se ha llevado a cabo para conocer el papel de la nitrofurantoína en la profilaxis de la infección del tracto urinario.

    Métodos: se llevó a cabo una revisión bibliográfica de la literatura científica en las principales bases de datos (Cochrane, Medline, Pubmed), limitando la búsqueda a los últimos 20 años y seleccionando aquellos artículos con mayor factor de impacto. A su vez se revisaron otro tipo de estudios como ensayos clínicos, revisiones bibliográficas y estudios de cohortes para evaluar la eficacia y seguridad de la nitrofurantoína.

    Resultados: La aparición de nuevas resistencias de E. Coli a distintos antibióticos de uso habitual ha hecho que la nitrofurantoína vuelva a incluirse en el tratamiento profiláctico de la ITU. La nitrofurantoína ha sido estudiada en los últimos años y se considera segura hasta los 12 meses. Su principal beneficio es que se excreta prácticamente en su totalidad por el tracto urinario, a diferencia de otros antimicrobianos que lo hacen por el intestino grueso. Sin embargo, su mayor desventaja es que presenta toxicidad, hepatotoxicidad, toxicidad pulmonar o neuropatía. No obstante, todas ellas tienen una incidencia muy baja. En los ensayos clínicos revisados la profilaxis fue efectiva y solo un paciente tuvo un efecto adverso grave (neumonía intersticial). Sí que hubo mayor incidencia de efectos adversos de carácter leve.

    Conclusiones: la nitrofurantoína es efectiva para la prevención de ITU. No obstante, su uso se asocia con un aumento en la aparición de efectos adversos de carácter leve. Se necesitan más estudios que comparen la nitrofurantoína frente a cotrimoxazol y fosfomicina trometanol.

    Objectives: nitrofurantoin has been used for the prevention of urinary tract infection (UTI) for over 60 years. In the last 20 years new resistances have appeared, therefore this drug is considered nowadays prophylaxis of choice. This bibliographic review was conducted to describe nitrofurantoin’s role in urinary tract infection prophylaxis.

    Methods: we performed a bibliographic review in the main database (Cochrane, Medline, Pubmed), restricting the research to the last 20 years, and selecting articles with the higher impact factor. We also considered clinical trials, reviews and cohort studies in order to evaluate nitrofurantoin´s efficacy and safety.

    Results: In the last 20 years new resistances have appeared, therefore this drug is considered nowadays prophylaxis of choice. Nitrofuratoin has been studied the past few years and it is supposed to be safe until 12 months of treatment. Its main benefit is that it is metabolized by the urinary tract, unlike other antimicrobials. However, a drawback is its hepatotoxicity, lung toxicity and neuropathy. Nevertheless, the incidence is very low. In the clinical trials considered, the prophylaxis was effective and just one patient suffered a serious adverse effect (interstitial pneumonia). The incidence of mild adverse effects was higher.

    Conclusions: nitrofurantoin is effective for the prevention of UTI. Nevertheless, its use is associated with a higher risk of non-severe adverse effect. Further studies are needed to compare nitrofurantoin with ciprofloxacin or fosfomycin trometamol.
