Manejo en cirugía oral del paciente con tratamiento anticoagulante o antigregante

    Actualmente parece que cada día es mayor la cantidad de personas que llegan a nuestra consulta y refieren que están bajo tratamiento anticoagulante o antiagregante por prevención o como terapia de alguna patología: cardiopatía isquémica, prótesis valvulares, trombosis venosas, etc. Si estos pacientes van a requerir algún tipo de cirugía oral se nos suelen plantear siempre las mismas dudas: ¿podemos suspender la terapia corriendo el riesgo de trombosis?, y ¿cuántos días antes la suspendemos?, o por el contrario,¿continuamos con la medicación con el posible sangrado excesivo durante y después de la cirugía? En este artículo revisamos la literatura e intentamos dar respuesta a estas preguntas, comentando las distintas pautas que podemos seguir.

    It seems clear that there are a big amount of people nowadays, who come to our consultation and they have some kind of oral anticoagulant or antiplatelet therapy as prevention or as treatment of pathologies like ischemic heart disease, prosthetic heart valves, venous thrombosis. If these patients require some kind of oral surgery, the same doubts are always asked: ¿can were remove therapy assuming the risk of thrombosis?, and how many days before surgery we can remove it? Or on contrary, do we go ahead with the medication with the possibility of an excessive bleeding while and after surgery? In this article we review the literature and try to give an answer to these questions, relating the different ways that have been suggested for these patients.
