Introducción: La adherencia al tratamiento (AT) resulta un fenómeno relevante a investigar en los sujetos que viven con una enfermedad crónica degenerativa, en el cual intervienen varios factores tanto intrínsecos como extrínsecos de cada sujeto; en donde se incluye a la familia como factor extra personal como red de apoyo afectando positiva o negativamente acorde a su funcionalidad, interviniendo en esta última la cohesión familiar (CF) y adaptabilidad (AF) familiar.
Objetivo: Analizar la relación de cohesión y adaptabilidad familiar con adherencia al tratamiento en sujetos con diabetes mellitus 2 de 50 a 70 años de edad.
Métodos: Estudio transversal analítico. En sujetos con DM2 de 50 a 70 años de edad en tratamiento farmacológico, de UMF No. 41 (CDMX). Instrumentos: Cédula de recolección de datos, FACES-III y MMAS-8. Análisis estadístico descriptivo y analítico determinando asociación mediante Razón de Momios de Prevalencia (RMP), significancia del 95% entre AT con CF, AF y otras variables sociodemográficas y clínicas.
Resultados: De los 140 sujetos estudiados el 70.7% fueron no adherentes, 40.0% cohesión aglutinada, 54.3%, adaptabilidad caótica, 46.4% familia con funcionalidad intermedia. La AT con los extremos disfuncionales de CF y AF no presentaron asociación, mientras que con escolaridad tuvo RMP 4.64 (p=0.001) y comorbilidad RMP 0.22 (p=0.007).
Conclusiones: La no relación con CF y AF puede explicarse a que la AT es mayormente influenciada por factores intrínsecos que intervienen en la autogestión de cuidado de cada sujeto y no tanto por factores interpersonales, tomando en cuenta que la familia pone en marcha mecanismos de autorregulación que le permiten seguir funcionando y ser una red de apoyo positiva. Son pocos los estudios sobre este fenómeno y carecen de temporalidad, por tanto, esta investigación da apertura a continuar estudiándolo, mediante estudios longitudinales.

Methods: Analytical cross-sectional study. In subjects with DM2 from 50 to 70 years of age in pharmacological treatment, from UMF No. 41 (CDMX). Instruments: Data collection card, FACES-III and MMAS-8. Descriptive and analytical statistical analysis determining association by Prevalence Odds Ratio (ORM), 95% significance between AT with CF, AF and other sociodemographic and clinical variables.
Results: Of the 140 subjects studied, 70.7% were non-adherent, 40.0% agglutinated cohesion, 54.3% chaotic adaptability, 46.4% family with intermediate functionality. The AT with the dysfunctional extremes of CF and AF did not present an association, while with schooling it had RMP 4.64 (p=0.001) and comorbidity RMP 0.22 (p=0.007).
Conclusions: The non-association with CF and AF can be explained by the fact that TA is mainly influenced by intrinsic factors that intervene in the self-management of care of each subject and not so much by interpersonal factors, considering that the family implements self-regulation mechanisms. that allow you to continue to function and be a positive support network. There are few studies on this phenomenon, and they lack temporality, therefore this research opens the way to continue studying it, through longitudinal studies.
