Ejes para una reforma al Sistema General de Seguridad Social en Salud de Colombia

    Recientemente, se ha discutido la necesidad de reformar el Sistema General de Seguridad Social en Salud. Sin embargo, a pesar de las distintas iniciativas radicadas ante el Congreso de Colombia, ninguna ha prosperado. Uno de los motivos aparentes de este fracaso es que el debate público respecto de cuáles son los cambios que se requieren es complejo: requiere abordar temas de economía, derecho y ética del cuidado. Por ello, en el presente artículo, se presenta un panorama integral basado en cuatro problemáticas: 1) Los indicadores de salud 2) Contradicción normativa 3) La estructura financiera y 4) El Sistema General de Participaciones. Posteriormente se plantean las estrategias propuestas de cambios en el actual sistema de salud que permitan balancear la ética del cuidado, la protección del derecho fundamental a la salud, y los aspectos económicos y fiscales del sistema. Las propuestas se centran en tres grandes aspectos, a saber: 1) Modelos diferenciados de acuerdo a los territorios; 2) Organización normativa del sector en un código sanitario y 3) Identificación del centro de financiación del sistema de salud.

    In recent years, different legislative proposals to reform the General System of Social Security in Health have been introduced in the Colombian Congress without success. The public debate regarding the specific changes needed is complex and consists of different perspectives that arise from the fields of the economy, law and ethics of care. In this article we present a comprehensive overview based on four core problems: 1) Health indicators 2) Regulatory contradiction 3) Financial structure and 4) The subnational transfers system. Afterwards, we present the proposed strategies for changes in the current Colombian health system, which allow balancing the ethics of care, the protection of the fundamental right to health, and the economic and fiscal aspects of the system. The proposals focus on three major aspects: 1) Differentiated models according to the territories; 2) Regulatory organization of the sector in a health regulatory codification and 3) Identification of the financing centre of the health system.
