Situación epidemiológica de la hepatitis A en España. Años 2021 y 2022

    Introducción: El virus de la hepatitis A, que produce una infección aguda del hígado, tiene como vías de transmisión la ingestión de agua o alimentos o el contacto directo persona-persona (especialmente entre contactos estrechos, como convivientes u otros contextos específicos como guarderías u hombres que tienen sexo con hombres - HSH). El objetivo de este estudio fue describir la evolución temporal y las características epidemiológicas de la hepatitis A en los años 2021 y 2022 en España, comparándolos con los datos de años previos (periodo 2015-2020).

    Método: Se analizaron los casos de hepatitis A notificados a la Red Nacional de Vigilancia Epidemiológica (RENAVE) durante el periodo 2015-2022, calculándose las incidencias acumuladas (IA) anuales, global y por sexo y grupos de edad. También se describieron las principales características de los brotes de los años 2021-2022.

    Resultados: La IA en los años 2021 y 2022 fue de 0,41 y 0,53 respectivamente, inferiores a las notificadas en años previos. No se observaron grandes diferencias en la distribución entre hombres y mujeres. En el 2021, el grupo de población que presentó mayores IA fueron los adultos > 44 años, mientras que en el año 2022 fue la población de 0-14 años. En los años 2021 y 2022 la IA en población menor de 44 años fue inferior comparando con el 2015, mientras que para la población mayor de esa edad no se observaron grandes diferencias.

    Conclusiones: Las IA de hepatitis A durante los años 2021-2022 fueron las mínimas registradas durante el periodo de estudio, con un descenso importante con respecto a años previos especialmente en población infantil y en individuos de 15 a 44 años, posiblemente debido a cambios en la epidemiología de la enfermedad por la pandemia de COVID-19 y la situación excepcional del brote de los años 2016-2017.

    Introduction: The hepatitis A virus, which causes an acute liver infection, is transmitted by ingestion of food and water and direct person-to-person contact, especially among close contacts (cohabitants or other specific contexts such as kindergartens or men who have sex with men - MSM). The aim of this study was to describe the temporal trends and epidemiological characteristics of hepatitis A in 2021 and 2022 in Spain, comparing to data from previous years (2015-2020).

    Method: We analysed the cases of hepatitis A reported to the National Epidemiological Surveillance Network (RENAVE in Spanish) during the 2015-2022 period, calculating the annual cumulative inci-dences (CI), globally and by sex and age groups. The main characteristics of the hepatitis A outbreaks notified in 2021 and 2022 were also described.

    Results: In 2021 and 2022, the CI was 0.41 and 0.53 respectively, lower than those reported in previ-ous years. No large differences were observed in the distribution between men and women. In 2021, the population group with the highest CI was adults > 44 years, while in 2022 was the population aged up to 15 years. In 2021 and 2022, the CI in population up to 44 years was fewer compared to 2015, while no large differences were observed for the older population.

    Conclusions: In 2021 and 2022, the hepatitis A CI was the minimum notified during the study period, with a significant decrease compared to previous years, especially in children and in population aged 15 to 44, possibly due to changes in the epidemiology of the disease by the COVID-19 pandemic and the exceptional situation of the outbreak of the years 2016-2017.
