Resumen de Incidencia y principales indicaciones de cesárea en primigestas usuarias de un hospital de Machala

  La cesárea es aquel procedimiento quirúrgico al que se somete la gestante cuando el parto vía vaginal constituye un riesgo para el binomio madre e hijo. Esta intervención, realizada bajo indicaciones médicas precisas, ha permitido disminuir el porcentaje de muertes materno-infantiles. Pero desde hace algunas décadas, este tipo de partos se ha viralizado, en todo el mundo, alcanzando un auge sorprendente entre las gestantes, llegando incluso a superar los límites establecidos por la OMS. Por lo tanto, el objetivo de este estudio es determinar la incidencia e indicaciones de cesárea en primigestas usuarias de un hospital general de la ciudad de Machala, durante el primer semestre del 2022. Para esto se realizó un estudio de enfoque descriptivo, cuantitativo y retrospectivo, en el periodo enero – junio del 2022, con una muestra total de 921 partos, incluyendo a gestantes de todas las edades y excluyendo a aquellas que se realizaron otros procedimientos ginecológicos. Los resultados demostraron que la incidencia de cesáreas en primigestas fue de 29.5% y entre las indicaciones más frecuentes el parto prolongado 20,3%, compromiso del bienestar fetal 16,7%, ruptura prematura de membranas 15,4% entre otros. El total de partos por cesárea realizados a primigestas en este establecimiento de salud, supera lo recomendado por la OMS. Por lo tanto, es necesario el trabajo en todos los niveles de atención para disminuir los riesgos en el embarazo y evitar las cesáreas innecesarias.

  The cesarean section is that surgical procedure to which the pregnant woman undergoes when the vaginal delivery constitutes a risk for the mother and child pairing. This intervention, carried out under precise medical indications, has made it possible to reduce the percentage of maternal and infant deaths. But for a few decades, this type of childbirth has gone viral throughout the world, reaching a surprising boom among pregnant women, even exceeding the limits established by the WHO. Therefore, the objective of this study is to determine the incidence and indications for caesarean section in primiparous users of a general hospital in the city of Machala, during the first semester of 2022. For this, a descriptive, quantitative and retrospective study was carried out. , in the period January - June 2022, with a total sample of 921 deliveries, including pregnant women of all ages and excluding those who underwent other gynecological procedures. The results showed that the incidence of cesarean sections in primiparous women was 29.5% and among the most frequent indications prolonged labor 20.3%, compromised fetal well-being 16.7%, premature rupture of membranes 15.4%, among others. The total number of deliveries by cesarean section performed on primiparous women in this health facility exceeds what is recommended by the WHO. Therefore, work is necessary at all levels of care to reduce the risks in pregnancy and avoid unnecessary cesarean sections
