Resumen de Caso clínico. Proceso de atención de enfermería a una paciente con sobrecarga del rol del cuidador

  La Esclerosis Lateral Amiotrófica o ELA es una enfermedad de causa desconocida del Sistema Nervioso Central que afecta a las neuronas motoras. Estas mueren o degeneran hasta que dejan de enviar estímulos a los músculos provocando atrofia y fasciculaciones, esto provoca que poco a poco el paciente deje de poder realizar sus funciones básicas por el mismo, como hablar, vestirse, deglutir, moverse y hasta respirar1.

  Caso clínico de una paciente de 65 años, hace dos años su marido fue diagnosticado de una Esclerosis Lateral Amiotrófica de inicio bulbar, desde entonces se ocupa de su cuidado las 24 horas al día, refiere ayuda por parte de sus hijos e incluso en la fase terminal, dispuso de ayuda de un tercer cuidador externo al ámbito familiar. La paciente acude a consulta refiriendo “no poder más” se siente agotada y sin fuerzas. Su marido ya no tiene movilidad en ninguna de sus extremidades, porta una gastrostomía para poder ser alimentado, y desde hace pocos meses respira mediante una BPAP. La paciente se siente capaz de manejar todos los aparatos ya que está apoyada por su familia sobre todo por su nuera, enfermera, pero refiere tener mucho miedo y ansiedad a la posibilidad de acontecimientos urgentes como un atragantamiento o una caída en algún cambio de postura. Verbaliza estar muy cansada física y psicológicamente, ya que su marido requiere de su ayuda las 24 horas del día, para poder cubrir sus necesidades básicas.

  Amyotrophic Lateral Sclerosis or ALS is a disease of unknown cause of the Central Nervous System that affects motor neurons. These die or degenerate until they stop sending stimuli to the muscles causing atrophy and fasciculations, this causes the patient to gradually stop being able to perform basic functions by himself, such as speaking, dressing, swallowing, moving and even breathing1.

  Clinical case of a 65 year old patient, two years ago her husband was diagnosed with bulbar onset Amyotrophic Lateral Sclerosis, since then she has been taking care of him 24 hours a day, she refers to help from her children and even in the terminal phase, she had the help of a third caregiver outside the family environment. The patient comes to the clinic saying she «can’t take it anymore», she feels exhausted and without strength… her husband has no mobility in any of his limbs, he has a gastrostomy to be fed, and for the last few months he has been breathing with a BPAP. The patient feels capable of handling all the equipment since she is supported by her family, especially by her daughter-in-law, a nurse, but she reports being very afraid and anxious about the possibility of urgent events such as choking or a fall during a change of position. She reports being very tired physically and psychologically, since her husband requires her help 24 hours a day to cover his basic needs.
