Resumen de Reflexiones y experiencias acerca de la reforma psiquiátrica andaluza

Tras 40 años de reforma psiquiátrica andaluza (década de 1980), comparto reflexiones y algunas de las experiencias vividas durante ese proceso en la provincia de Cádiz. Comienzo aludiendo a los efectos indeseables de la institución manicomial, que contextualizo con reseñas historicistas relativas a la asistencia psiquiátrica. En buena lógica, a esto le sigue la evocación al cómo se fue desarrollando la transformación asistencial, que nos llevó a implantar el modelo de intervención comunitario mientras desintitucionalizábamos y externábamos a los internados hasta cerrar el manicomio provincial. Luego sostengo que la reforma, en lo cualitativo y cuantitativo, ha sido positiva y aludo a algunos de los logros que así lo atestiguan. Sin embargo, han quedado cosas por hacer por nuevas crisis socioeconómicas y nuevas voluntades políticas, que operan como condicionantes de la salud (mental). Además, esto nos impone un tiempo de zozobra que también se nutre con la primacía del psicofármaco y del biologicismo en menoscabo de las tecnologías psicoterapéuticas, entre las que están los cuidados psicosociales excelentes. Cierro el manuscrito con interrogantes: ¿acaso quieren devolver a la población andaluza al modelo de beneficencia?, ¿peligra el modelo comunitario y/o biopsicosocial?, ¿cuáles deben ser los compromisos inmediatos de las enfermeras de salud mental?

After 40 years of Andalusian psychiatric reform (1980s), I share my reflections and some of the experiences I lived during this process in the province of Cadiz. I begin by alluding to the undesirable effects of the institution of the psychiatric hospital, which I contextualise with historical overviews of psychiatric care. Logically, this is followed by an evocation of how the transformation of care developed, which led us to implement the model of community intervention while we deinstitutionalised and externalised the inmates until the provincial psychiatric hospital was closed. I then argue that the reform, both qualitatively and quantitatively, has been positive and point to some of the achievements that attest to this. However, new socio-economic crises and new political wills, which operate as conditioners of (mental) health, have left some things to be done. Moreover, this imposes on us a time of anxiety that is also nourished by the primacy of psychotropic drugs and biologicism to the detriment of psychotherapeutic technologies, including excellent psychosocial care. I close the manuscript with some questions: do they want to return the Andalusian population to the welfare model? is the community and/or biopsychosocial model in danger? what should be the immediate commitments of mental health nurses?
