Resumen de El proceso de adherencia al tratamiento por tuberculosis: aportes desde el trabajo social

El artículo es producto del trabajo realizado en la Comisión Sanitaria de Control de Tuberculosis de la Provincia de Córdoba, República Argentina Su objetivo es problematizar el concepto de adherencia al tratamiento para superar la mirada positivista, identificar determinantes sociales que intervienen en el proceso de salud-enfermedad-atención que influyen durante el tratamiento y describir los aportes del Trabajo Social en este proceso. Se considera que un tratamiento integral debe incorporar intervenciones que mitiguen los determinantes que obstaculizan la culminación del tratamiento o que impidan ver las verdaderas causas de los fracasos. En Córdoba, la Ley 9185/04 brinda apoyo económico como parte de un tratamiento integral de salud; a partir de ella se diseñan estrategias con el fin de favorecer la adherencia de pacientes que ven obstaculizado su acceso al sistema de salud.
A través del análisis cualitativo de las entrevistas de admisión al Programa del año 2017, obtenidas en los centros hospitalarios y de atención primaria de capital e interior de Córdoba, se concluye que la adherencia es más que el cumplimiento de la indicación, implica un proceso por el que sujetos conscientes de su situación de salud, se empoderan de su derecho al acceso a la misma y lo hacen valer, a través del uso de los dispositivos dispuestos para tal fin, desde sus propias particularidades. En el proceso de adherencia al tratamiento por tuberculosis, resulta relevante destacar la relación paciente – equipo de salud, la cual será determinante en la culminación del mismo.
Para el análisis de los datos se utilizó la información que proviene de las entrevistas de admisión al Programa. En el año 2017 ingresaron 84 solicitudes de transferencias formales y de esas entrevistas semi estructuradas se realizó el análisis cualitativo.
A lo largo del ensayo el término “paciente” es utilizado como sinónimo de las personas bajo tratamiento, usuarias/os de los servicios o ciudadanas/os.

The article is the product of the work carried out in the Sanitary Commission of Tuberculosis Control of the Province of Córdoba, Republic of Argentina. Its objective is to problematize the concept of adherence to treatment in order to overcome the positivist view, to identify social determinants that intervene in the health-disease-attention process that influence during treatment and to describe the contributions of Social Work in this process. It is considered that an integral treatment must incorporate interventions that mitigate the determinants that hinder the culmination of the treatment or that prevent seeing the real causes of the failures. In Córdoba, Law 9185/04 provides economic support as part of comprehensive health treatment; strategies are designed to promote adherence of patients whose access to the health system is hindered. Through the qualitative analysis of the interviews of admission to the 2017 Program, obtained in the hospitals and primary care centers of the capital and interior of Cordoba, it is concluded that adherence is more than compliance with the indication, it implies a process by which subjects aware of their health situation, empower themselves of their right to access to it and make use of it, through the use of devices provided for that purpose, from their own particularities. In the process of adherence to treatment for tuberculosis, it is important to highlight the patient-health team relationship, which will be decisive in the culmination of the treatment. For the analysis of the data we used the information that comes from the interviews of admission to the Program. In 2017, 84 formal transfer requests were received, and qualitative analysis was carried out from these semi-structured interviews. Throughout the trial, the term "patient" is used as a synonym for people under treatment, service users, or citizens.
