La encefalitis del tronco encéfalo de Bickerstaff es un síndrome raro definido por la triada clínica de oftalmoplejía, ataxia y disminución del nivel de consciencia. En ocasiones aparece superpuesta con el síndrome de Guillain-Barré, asociado tetraparesia y debilidad flácida, con recuperación funcional más lenta e incluso a veces parcial, suponiendo una gran discapacidad para el paciente. El diagnóstico va a ser fundamentalmente clínico, apoyándose en la presencia de anticuerpos anti GQ1b así como en los hallazgos en las pruebas de neuroimagen. La inmunoglobulina humana por vía intravenosa es el tratamiento que ha mostrado mayor efectividad en la recuperación del paciente. En este artículo se presenta el caso de un varón de 52 años que ingresó en el servicio de Neurología por presentar clínica compatible con romboencefalitis de Bickerstaff, confirmándose posteriormente con pruebas complementarias y buena respuesta al tratamiento.

Bickerstaff brainstem encephalitis is a rare syndrome defined by the clinical triad of ophthalmoplegia, ataxia, and decreased level of consciousness. Sometimes it appears overlapping with Guillain-Barré syndrome, associating tetraparesis and flaccid weakness, with slower and sometimes even partial functional recovery, creating a great disability for the patient. The diagnosis will be fundamentally clinical, based on the presence of anti-GQ1b antibodies as well as the findings in the neuroimaging tests. Intravenous human immunoglobulin is the treatment that has shown the greatest effectiveness in a patient’s recovery. This article presents the case of a 52-year-old man who was hospitalized to the Neurology service due to symptoms compatible with Bickerstaff’s rhombencephalitis. The fact was confirmed later based on complementary tests and a good response to treatment.
