  La leucemia basofílica aguda (Clasificación OMS-2008-Leucemia mieloblástica aguda- NOS) es una entidad rara que corresponde a menos del 1% de todos los casos de leucemia mieloblástica aguda(1,2). Se presenta un paciente masculino de 70 años con pérdida de peso, esplenomegalia y leu-cocitosis 90.000/mm3. Presentaba blastos de tamaño mediano con granularidad presente y escasa en sangre periférica (80%). La citoquímica mostró mieloperoxidasa (MPO) negativa, ácido peryódico-Schiff (PAS) positivo débil, α naftil acetato esterasa (ANAE) negativa. El inmunofenotipo de médula ósea no permitió asignación de linaje: marcadores linfoides negativos, CD34+, HLA-DR ++, CD13 -/+, CD33-/+, CD 123+/++. Translocación (9;22) negativa. Con tinción con azul de toluidina se observó metacromasia, arribándose al diagnóstico de leucemia aguda con diferenciación basofílica.

  Acute basophilic leukemia (WHO-Classification 2008-Acute myeloblastic leukemia-NOS) is a rare disease that represents less than 1% of all the cases of acute myeloblastic leukemia. We report the case of a 70 year old man, who presented weight loss, massive splenomegaly and 90.000 leukocytes/mm3, with 80% of blasts of medium size and low number of granules. Cytochemistry reactions were negative for peroxidase and Naphtol AS D Esterase, and weakly positive for periodic acid–Schiff. Bone marrow’s immuno - phenotype didn’t allow lineage assignment: lymphoid markers were negative, HLA-DR++; CD13-/+; CD33-/+; CD34++; CD123+/++. Translocation t(9;22) was negative. A tolouidine blue staining showed a positive metachromatic reaction, allowing thus a diagnosis of acute leukemia with basophilic differentiation.
