La creciente prevalencia de personas con demencia ha puesto de manifiesto la importancia de la exploración cognitiva como un elemento fundamental para establecer la presencia y la gravedad del deterioro cognitivo, incluso orientando hacia la etiología más probable. El primer paso debe comprender una anamnesis y una exploración clínica completa, con el principal objetivo de descartar una causa reversible del deterioro cognitivo. Posteriormente, una evaluación neuropsicológica en mayor o menor profundidad según el ámbito clínico indicará el dominio cognitivo más afectado. Son de gran utilidad en una valoración inicial las pruebas de cribado como el Mini-Mental State Examination o el Montreal Cognitive Assessment que, si bien con limitaciones, son capaces de evaluar la afectación en la función ejecutiva y visuoespacial, atención, lenguaje y memoria. Posteriormente y en base a la sospecha clínica, la exploración cognitiva debe completarse con pruebas específicas de cada uno de los dominios anteriores.

The growing prevalence of individuals with dementia has highlighted the importance of the cognitive examination not only as an essential element for establishing the diagnosis and severity of cognitive impairment but also as a reliable indicator of the most probable etiology. The medical history and the clinical examination should be the first step, with the of ruling out a reversible cause of cognitive impairment. Then, a neuropsychological evaluation of greater or lesser depth according to the clinical scenario will indicate the most affected cognitive domain. Screening tests such as the Mini Mental State Examination or the Montreal Cognitive Assessment are useful tools for an initial evaluation as they examine the executive and visuospatial functions, attention, language, and memory. However, they have several limitations. Later, based on the results, the cognitive examination must be completed with specific tests for each of the aforementioned domains.
