Hallazgo inmunohematológico anticipala refractariedad al tratamiento con daratumumab en paciente con mieloma múltiple

    El daratumumab (DARA) es un anticuerpo monoclonal humano de especificidad anti-CD38, utilizado en el tratamiento de pacientes que padecen mieloma múltiple recaído/refractario. Es conocido que este medicamento interfiere en las pruebas inmunohematológicas séricas, positivizando en medio antiglobulínico tanto la investigación de anticuerpos irregulares como las pruebas de compatibilidad transfusional, desde el inicio del tratamiento hasta varios meses después de su finalización. Se describe el caso de una paciente de 65 años con diagnóstico de mieloma múltiple en recaída, que fue sometida a tratamiento con DARA. Se constató en sus pruebas inmunohematológicas séricas que se habían positivizado según es habitual, una prematura e inesperada negativización. La paciente fue refractaria al tratamiento con DARA. Se piensa que nuestro hallazgo inmunohematológico atípico podría haber estado vinculado al desarrollo de anticuerpos anti-DARA.

    Daratumumab (DARA) is a human monoclonal antibody that targets CD38. It is used for the treatment of patients with refractory/relapse multiple myeloma. DARA interferes with immunohematological tests, causing positive agglutination reactions in indirect antiglobulin tests, antibody screening tests and crossmatches, from the beginning of the treatment until several months after finished.We describe the case of a 65-years old patient with relapsed multiple myeloma treated with DARA. It was confirmed positivization in its immunohematological tests as planned, but evolving with a premature and unexpected negativization. The patient was refractory to this treatment. This atypical immunohematological finding could be due to the development of anti-DARA antibodies.
