Se hace una breve descripción del funcionamiento del sistema inmunitario con especial énfasis en el papel de las células dendríticas en la estimulación de la respuesta inmunitaria y de la necesidad de que ésta sea tolerante con los antígenos del propio organismo y con los antígenos extraños “inocuos”. Seguidamente se introducen los conceptos de alergia, atopia, alérgeno y otros, y se describe la secuencia de acontecimientos que tienen lugar durante el desarrollo de la respuesta alérgica mediada por IgE y el papel desempeñado por las células implicadas: células dendríticas, linfocitos T y B, mastocitos, basófilos, eosinófilos y células T reguladoras, así como de los factores solubles liberados por estas células y que intervienen en su desarrollo. Se describen a continuación los factores genéticos que predisponen a la atopia y, finalmente, se describen los efectos clínicos de las reacciones alérgicas y las estrategias que se vienen utilizando para su tratamiento.

The present article briefly describes the functioning of the immune system with special emphasis on the role of dendritic cells in stimulating the immune response and on the need for this response to tolerate body's antigens and “innocuous” foreign antigens. Next, we describe the concepts of allergy, atopy, and allergen, among others, and the sequence of events that take place during the development of the IgE-mediated allergic response and the role played by the cells involved: dendritic cells, T and B lymphocytes, mastocytes, basophils, eosinophils and regulatory T cells, as well as the soluble factors released by these cells and which intervene in their development. The genetic factors predisposing to atopy are outlined. Finally, the clinical effects of allergic reactions and the strategies that have been used for their treatment are described.
