Resumen de Grave complicación cardíaca secundaria a traumatismo cerrado de tórax

  Se presenta el caso de una rara y grave complicación en el traumatismo cerrado de tórax.

  Paciente de 42 años derivado al centro de los autores por politraumatismo secundario a accidente de tráfico. Evolucionó durante la internación con empeoramiento clínico, grave inestabilidad hemodinámica, confirmando por ecocardiografía transtorácica y transesofágica una gran comunicación interventricular (CIV) de tipo muscular, asociada a insuficiencia significativa tanto de válvula mitral como de válvula tricuspídea. Se efectuó cirugía correctiva con parche de la CIV y plástica tricuspídea con ominosa evolución postoperatoria.

  Se resalta el rol de la ecocardiografía transtorácica y transesofágica en esta patología.

  We present a rare and severe complication following blunt chest trauma.

  A 42 year-old patient was transferred to our medical center after suffering a motorcycle crash accident with several trauma injuries. During the hospitalization he presented worsening clinical and hemodynamic conditions. After performing both transthoracic and transoesophageal echocardiography we confirmed a large interventricular septal defect plus severe mitral and tricuspid valves regurgitation. Corrective surgery of the defect and tricuspid valvuloplasty was made with ominous result.

  We highlight the importance of the echocardiography in the urgent setting of patients who have suffered of a blunt chest trauma.
