The infection produced by the HIV has been converted into the epidemic of the XXth century. During the last 16 or 17 years, the AIDS, or final pathological state of such a way that the clinical and serological markers of the disease may have changed. At the beginning, clinical manifestations of the disease, as oropharingeal pseudomembranouscandidiasis, hairy leukoplakia of the tongue or the Kaposi's angiosarcoma, in any location, were predicting a next evolution to AIDS. This pathology was in intimate relationship to the level of CD4 cells, generally inferior to 200 for mi of blood in such situations. The different drugs (analogous to the nucleosides with inhibitoring action of the viral retrotranscriptase, first, and inhibitors of the viral protease, after), have permitted to reduce the aggressiveness of the infection. The advance in the diagnostica! tests has permitted to know, since 1995, the quantity of viral RNA copies per mi of blood (viral load). lt is very useful to know the efficiency of the treatments. These have gone evolving from moo.otherapy to the most efficient combined therapy. Thanks to these treatments, the viral load detections, sometimes of several mi Ilion of copies per mi, have been reduced at undetectable levels (currently are considered thus less than 80 viral RNA copies per mi of blood). Perhaps the approaching to the beginning of the end of the pandemic is near, at least in the developed countries, where it is possible to administer so costly drugs.
