Resumen de Proposed Improvement on Availability of Sarawak Marine Aids to Navigations by Using FSA And Swiss Cheese Model

  • The study started with obtaining aids to navigation maintenance data year 2014 to 2016 from MarineDepartment Sarawak Malaysia. The data is used to determine the availability percentage of MarineAids to Navigation (AtoN) in Sarawak according to type of equipment. The navigation buoy availabilitywas below the standard set according to IALA and QMS. Formal Safety Assessment (FSA) was usedto asses risk by unavailability of navigation buoy. Fault Tree Analysis was used to identify the rootcause of failure. Swiss Cheese Model (SCM) was used to identify the management related cause atvarious management level. Risk control options were proposed based on findings in the FTA andSCM. The best risk control option proposed to top management was based on the five-level cost andeffectiveness matrix of Cost-Benefit Analysis. Result of this research is applied to reduce the downtimeof navigational buoys and would improve the safety of navigation in Sarawak waters.
