Resumen de Análisis de incidentes de seguridad: estrategia para la implementación de medidas en el uso seguro de medicación en quirófano

  La seguridad clínica puede definirse como la ausencia de lesiones accidentales durante la asistencia sanitaria y abarca todas aquellas actividades dirigidas a evitar, prevenir o mitigar los resultados adversos que podrían ser consecuencia de la asistencia sanitaria. Se propone como Objetivo General aumentar la seguridad del paciente quirúrgico minimizando los posibles riesgos derivados del uso de medicación durante la cirugía.

  Se ha realizado un estudio observacional descriptivo retrospectivo a partir de la revisión y análisis de aquellos incidentes de seguridad relacionados con el uso de medicación en quirófano. Se tomaron como muestra las notificaciones registradas desde enero a diciembre de los años 2016 y 2017 por profesionales de enfermería y anestesiólogos. Extraídos de la base de datos generada por el grupo analizador de incidentes del bloque quirúrgico del Hospital de Valme de Sevilla.

  En total, en estos 2 años, las notificaciones relacionadas con incidentes de medicación en el área quirúrgica han sido 31 casos en global. De entre todas estas notificaciones de incidentes de seguridad, el 74,2% no produjeron daño al paciente y solo el 25,8% ocasionaron morbilidad menor.

  Se decide adoptar medidas que garanticen la seguridad del paciente y se opta por introducir en el área quirúrgica un sistema automatizado para la dispensación y control de medicación general, intravenosa y estupefacientes.

  Clinical safety can be defined as the absence of accidental injuries during healthcare and encompasses all activities aimed at avoiding, preventing or mitigating adverse outcomes that could be a consequence of healthcare. It is proposed as a General Objective to increase the safety of the surgical patient by minimizing the possible risks derived from the use of medication during surgery.

  A retrospective descriptive observational study has been carried out based on the review and analysis of those safety incidents related to the use of medication in the operating room. The notifications registered from January to December of the years 2016 and 2017 by nursing professionals and anesthesiologists were taken as a sample. Extracted from the database generated by the incident analyzer group of the surgical block of the Hospital de Valme in Seville.

  In total, in these 2 years, the notifications related to incidents of medication in the surgical area have been 31 cases globally. Among all these notifications of security incidents, 74.2% did not cause harm to the patient and only 25.8% caused minor morbidity.

  It was decided to adopt measures that guarantee the safety of the patient and it was decided to introduce an automated system in the surgical area for the dispensing and control of general, intravenous and narcotic medication.
