Resumen de Caso clínico en paciente tras un accidente cerebrovascular embólico

  El paciente sufre de una hemiparesia izquierda dada por un accidente cerebro vascular en el territorio de la arteria cerebral media derecha. Las pruebas nos muestran que el daño está anatómicamente más localizado en la corteza cerebral a nivel de la cápsula interna así que la vía cortico espinal está directamente afectada. Muestra una hipotonía axial además de una anteversión pélvica, lo que nos hace pensar que la vía retículo-espinal lateral está alterada.

  Como hipótesis se plantea que la hipotonía axial del paciente y la falta de control le impiden tener una buena estabilidad de la cintura escapular y pélvica. La falta de estabilidad de las cinturas le impiden ejecutar correctos movimientos y tener un adecuado reclutamiento motor de su brazo izquierdo.

  En cuanto al tratamiento se centrará en la extremidad superior, con el objetivo de ganar tono postural normal, normalizar patrones de movimiento y asi poder utilizarlo para las AVD

  The patient suffers from a left hemiparesis due to a cerebrovascular accident in the territory of the right middle cerebral artery. The tests show us that the damage is anatomically more localized in the cerebral cortex at the level of the internal capsule, so the corticospinal pathway is directly affected. It shows axial hypotonia as well as pelvic anteversion, which makes us think that the lateral reticulo-spinal pathway is altered. As a hypothesis, it is proposed that the patient’s axial hypotonia and lack of control prevent him from having good stability of the shoulder and pelvic girdle. The lack of stability of the waist prevents him from executing correct movements and having an adequate motor recruitment of his left arm. Regarding the treatment, it will focus on the upper limb, with the aim of gaining normal postural tone, normalizing movement patterns and thus being able to use it for ADL.
