En los últimos años, se han realizado numerosos intentos para lograr disminuir la mortalidad asociada al shock séptico, tanto desde el punto de vista del diagnóstico precoz como de optimización del tratamiento. Entre las novedades en el tratamiento se incluye el uso de vitamina C y tiamina asociado a hidrocortisona, habiéndose comprobado en varios estudios observacionales la mejoría de la supervivencia y la disminución del uso de vasopresores. Sin embargo, este nuevo ensayo clínico pone en duda el papel de la vitamina C y la tiamina, sin encontrar diferencias significativas entre la monoterapia con hidrocortisona y la combinación de las tres.
In recent years, numerous attempts have been made to reduce the mortality associated with septic shock, both from the point of view of early diagnosis and treatment optimization. New developments in the treatment include the use of vitamin C and thiamine associated with hydrocortisone, with several observational studies having been shown to improve survival and decrease in vasopressor use. However, this new clinical trial calls into question the role of vitamin C and thiamine, without finding significant differences between hydrocortisone monotherapy and the combination of all three.
