El silencio familiar es un tipo de respuesta que suele observarse con cierta frecuencia ante problemas de salud graves y con mal pronóstico. Tanto desde lo legal, lo bioético, incluso culturalmente, no se sostiene en una sociedad actual una respuesta de este tipo, pese a ello, sigue ocurriendo.
Se presenta un caso clínico de un paciente diagnosticado de cáncer de pulmón diseminado con hígado metastático, atendido en gestión de casos, y la reacción familiar ha sido ocultar el diagnóstico al paciente.
El abordaje del caso fue complicado, por la rápida evolución del paciente dentro de su proceso al final de la vida y la dificultad para poder trabajar con la familia.

Family's silence is a kind of response which can be often observed in the presence of fatal and bad prognosis illnesses. From the legal, bioethics or even cultural point of view, this behaviour is not sustained on our nowadays society. However this type of response is still common.
We present a clinic case of a patient that was diagnosed of spread lung cancer and hepatic metastasis that was attended by case management nursing. The family's reaction was to conceal the diagnosis to the patient. The approach of this case was complicated because of the fast outcome of the patient's process at the last stage of his life and the difficulty to work with his family.
