The pandemic is an interesting test to deeply understand how the Italian healthcare system has changed. Financial impoverishment and inequalities between regions have made it more difficult to respond to the pandemic. At the same time, the public nature of the system and the involvement of the community have been strengths. It is important to take the opportunity to return to talk about the healthcare system in Italy, without forgetting the political role of public health decisions.
