  La neurectomía puede ser el tratamiento defi nitivo del dolor inguinal crónico posoperatorio de tipo neuropático. Como posibles opciones existe la triple neurectomía y la neurectomía selectiva. La bibliografía sobre este tema es defi ciente.

  Nuestro objetivo es analizar ambas opciones, sus ventajes e inconvenientes.

  La triple neurectomía parece tener una tasa de efi cacia mayor, pero sus secuelas también pueden ser mayores por la denervación proximal. La selectiva es una opción basada en una prueba clínica, el mapeo por dermatomas, que puede ser poco resolutiva por las grandes variaciones en la distribución de los nervios inguinales. La decisión sobre el tipo de neurectomía no puede basarse en los datos de la literatura, debe estar basada en la experiencia del cirujano en el tratamiento del dolor crónico neural.

  Neurectomy may be the defi nitive treatment of postoperative chronic inguinal pain of the neuropathic type. Possible options include triple neurectomy and selective neurectomy. The literature on this subject is defi cient. Our objective is to analyze options, advantages and disadvantages.

  Triple neurectomy seems to have a higher effi cacy rate but its sequelae can also be greater due to proximal denervation.

  The selective one is an option based on a clinical test, the dermatome mapping, which can be unresolved due to the great variations in the distribution of the inguinal nerves. The decision on the type of neurectomy cannot be based on data from the literature; it must be based on the experience of the surgeon in the treatment of chronic neural pain.
