The aim of this study was to evaluate the prevalence of the mesiopalatine canal and the configuration of the mesiobuccal root by cone-beam computed tomography according to the Vertucci classification. A total of 70 upper first molars were evaluated in computed tomography that are part of the digital archive of a diagnostic imaging clinic in Parnaíba – PI – Brazil. Axial slices were performed at 5 and 10 mm from the apex and from the coronal and sagittal sections. The results revealed that the mesiopalatine canal is present in 47.1% of the cuts at 10 mm of the root apex and in 34.3% of the cuts at 05 mm of the apical limit. Regarding the morphology of the mesiobuccal root, according to the Vertucci classification, the most frequent forms were: type I (38.6%) and type II (28.6%). Cone-beam commputed tomography should be considered as a means of complementary diagnosis, in cases of difficult location of the mesiopalatine canal.
