    • Cortical bio p s ies o f 11 p a ti ents with tr a umatic brain oede m a were consecutive ly studied by lig ht mic rosc o p y (LM) us in g thick plastic secti o n s, scanning -tra ns missio n e lectron microscopy ((S )T E M ) using semithin pl as tic sectio ns and trans missio n e lectro n microscop y (T E M ) us in g ultrathin sectio ns . Samples were gluta ra ld e hy de-osmium fixed a nd embedde d in A raldite or E p o n . Thic k sectio ns w e re sta in e d with toluidine -blue fo r lig ht microscop y. S emithin sectio ns were ex amine d uns ta in ed a nd uncoated for (S ) T EM. U ltra thin secti o ns were sta in ed w ith ura ny l a nd lead. P e rivascul a r haem orrhages and p e rivascula r extravasati o n o f pro te inaceou s oed e m a fluid w e re o bserve d in b oth m o d e r a te and s evere oed e m a . Ischae mic p y ramida l a nd n on-py ramida l ne rve cells appeared s hrunke n, e lectro n de nse a nd w ith e nl a rge me nt of intracyto plasmic m embra ne compa rtme nt. No ta bl y swolle n astrocytes wer e observed in a ll samples examine d . Glycogen-ric h and g lycog en-deple te d as trocytes w e re identified in a noxic-ischacmic regio ns . D ark and h ydropic sat ellite, interfascic ula r and perivascula r oligode ndrocytes we re also found. The statu s spo ng io s us of severe ly oede ma to us brain p a r e n c h y m a observed by LM a n d (S ) T E M was corre late d w ith th e enl a rged extracellula r space a nd dis rupted ne uro pil observed b y T E M. The (S)T E M is recommend e d as a suitable technique fo r study in g patho logical pro cesses in the central ne rv o us syste m a nd as an info rmative adjunc t t o LM a nd TEM.
