Resumen de Barreras del servicio de dispensación de medicamentos y productos sanitarios de la farmacia comunitaria y propuestas de mejora

  La dispensación de medicamentos es un servicio básico que las farmacias comunitarias llevan prestando a la población española desde su origen. En el último tercio del siglo xx la nueva filosofía de práctica farmacéutica conocida como atención farmacéutica modifica significativamente los objetivos de este servicio y, por tanto, la forma en la que éste se debería prestar. La dispensación es un servicio complejo que aglutina la realización de verificaciones clínicas y administrativas sobre el estado de salud del paciente y los medicamentos que utiliza y le han prescrito. Para alcanzar sus objetivos es necesario adaptar este servicio a las necesidades asistenciales surgidas como consecuencia del uso generalizado de los medicamentos y productos sanitarios, a las nuevas tecnologías y a la forma de llevarla a cabo, sin olvidar el papel de los técnicos en Farmacia y Parafarmacia. El presente trabajo revisa y comenta los elementos indispensables del diseño de este servicio establecidos en el Consenso sobre Atención Farmacéutica de 2001, así como las barreras que todavía hoy hay que vencer para quitar alcanzar sus objetivos. El trabajo realiza cinco propuestas de mejora: la modificación del sistema retributivo, el diseño de indicadores, la adaptación de la estructura física y funcional de la farmacia, la incorporación del concepto de validación de la dispensación y la asignación de funciones entre el farmacéutico y el técnico en Farmacia y Parafarmacia. Esta última propuesta se acompaña de un análisis de los procesos y subprocesos del procedimiento del servicio para facilitar la asignación de funciones y responsabilidades entre ambos.

  Drugs dispensing is a basic service offered by community pharmacies to the Spanish population since it was first established. In the last third of the 20th century, the new approach of pharmaceutical practice known as pharmaceutical care significantly changed the objectives of this service, hence the shape it should take. Drugs dispensing is a complex service that encompasses the realization of clinical and administrative testing on the patient’s health condition and the drugs they use and were prescribed.

  To attain its objectives it is necessary to adapt this service to the assistance needs as a result of the generalized use of drugs and health products, to the new technologies and the form in which it should be carried out, without forgetting the role of Pharmacy and Parapharmacy technicians. This article reviews and comments the key aspects of the design of this service, established in the 2001 Pharmaceutical Care Agreement, and the barriers that still have to be overcome today to attain its targets. The work presents five proposals for improvement: the modification of the payment plan, the design of indicators, the adaptation of the pharmacy’s physical and functional structure, the integration of the concept of validation and dispensing and assigning functions between the pharmacist and the technician in Pharmacy and Parapharmacy. The latter proposal will be accompanied by an analysis of the processes and sub-processes of the procedures of the service to facilitate the assignment of functions and responsibilities between the two.
