Niveles de conocimientos y percepción con que los médicos pasantes administrarán el plan básico de salud (PBS) en las unidades de atención primaria (UNAP), período enero-agosto 2003

    En la República Dominicana el Sistema Nacional de Salud ha sido deficiente por años. Con la aprobación de la Ley de Seguridad Social (8701) y ley general de salud (42-01) se ha planteado la reforma y modernización del sector salud por las limitaciones y precariedades que han existido y que existen hasta la fecha. La presente investigación se enmarcó en determinar el nivel de conocimiento y percepción que poseen los médicos pasantes presentes y futuros directores en la administración de las Unidades de Atención Primaria (UNAP), así como también determinar el nivel de conocimiento y percepción con que han de administrar la cartera de servicios a ofrecer en el Plan Básico de Salud (PBS) durante el periodo Enero-Agosto 2003. Para esto se tomó una muestra de 120 médicos pasantes a los cuales se les aplicó una entrevista de 16 preguntas abiertas en las diferentes zonas geográficas del país. La mayoría de los conocimientos expresados por los médicos pasantes resultaron ser de muy bajo porcentaje en lo concerniente a las UNAP, PBS y nueva Ley de Seguridad Social, puesto que el alcance de los centros de primer nivel solo es conocida por un 16%, más de la mitad de los entrevistados posee un bajo conocimiento y tres de cada diez no conocen el área de alcance de las UNAP. Respecto al PBS solo un 1 % posee un concepto claro del mismo, el 99% restante posee una idea vaga o desconoce el término, en cambio a manera de contraste se observó que la percepción acerca del PBS y las UNAP posee otro punto de vista, el 33% es optimista en cuanto al funcionamiento, el 36% no contestó la pregunta formulada, un 12% posee sus dudas y se mostró renuente al tema tratado, un 12% se tomó pesimista mientras que un 7% señala no saber nada respecto al tema. Los conocimientos expresados fueron en gran medida de porcentajes muy bajos, sin embargo la percepción respecto a los temas de cómo se han de desarrollar y los beneficios que ha de ofrecer cada uno de estos elementos, resultaron ser en gran medida altos, y es por tal motivo que las recomendaciones a ofrecer en la presente investigación van dirigidas hacia campañas propiamente de capacitación hacia los médicos pasantes por ser estos los presentes y probables futuros directores de las UNAP y de entregar a cada uno de los grupos poblacionales la cartera de servicios ofrecidos dentro del PBS. A su vez los conocimientos expresados no fueron satisfactorios en cuanto al dominio del tema.

    In the Dominican Republic, the National Health System has been deficient for years. With the approval of the Social Security Law (8701) and general health law (42-01), the reform and modernization of the health sector has been considered due to the limitations and precariousness that have existed and that exist to date. The present investigation was framed in determining the level of knowledge and perception that present and future directors have in the administration of the Units of Primary Attention (UNAP), as well as to determine the level of knowledge and perception with which they have to administer the portfolio of services offered in the Basic Health Plan (PBS) during the period January-August 2003. For this, a sample of 120 medical interns was taken, to which an interview of 16 open questions was applied in the different geographical areas. from the country. Most of the knowledge expressed by the medical doctors turned out to be of very low percentage regarding the UNAP, PBS and new Social Security Law, since the reach of the first level centers is only known by 16%, more Half of the interviewees have low knowledge and three out of ten do not know the scope of the UNAP. Regarding the PBS only 1% has a clear concept of the same, the remaining 99% has a vague idea or ignores the term, instead as a contrast it was observed that the perception about the PBS and the UNAP has another point of view, 33% are optimistic about the operation, 36% did not answer the question, 12% have doubts and were reluctant to address the issue, 12% took pessimistic while 7% said they do not know anything about the theme. The knowledge expressed was, to a large extent, very low, but the perception regarding the subjects of how they should be developed and the benefits that each of these elements has to offer, turned out to be largely high, and is therefore reason that the recommendations to offer in the present investigation are directed towards proper training campaigns for the medical interns because these are the present and probable future directors of the UNAP and to deliver to each one of the population groups the portfolio of services offered within the PBS. In turn, the knowledge expressed was not satisfactory in terms of mastery of the topic.
