Resumen de Cuidados Paliativos y rehabilitación. Experiencia en un hospital de media y larga estancia

  Objetivo: describir el perfil de los pacientes que han sido subsidiarios de tratamiento rehabilitador en la unidad de Cuidados Paliativos durante los últimos 5 años. Describir las indicaciones de tratamiento rehabilitador, así como las principales terapias utilizadas. Disponer de datos aproximados sobre la efectividad del tratamiento rehabilitador.

  Método: estudio descriptivo retrospectivo. Revisión de las interconsultas realizadas al servicio de rehabilitación desde la unidad de cuidados paliativos, entre enero de 1997 y diciembre de 2001.

  Resultados: se revisaron 1.172 interconsultas de las cuales se seleccionaron 33 (2,81%) remitidas desde la unidad de cuidados paliativos. De los 33 pacientes, 23 iniciaron rehabilitación (69,7%). Los principales diagnósticos de rehabilitación fueron: alteraciones motoras e impotencia funcional secundaria a atrofia muscular por encamamiento. La técnica de tratamiento más empleada fue la cinesiterapia. El número medio de sesiones de rehabilitación que se realizaron fue de 6. Se consiguieron los objetivos terapéuticos en un 60,9% de los casos.

  Conclusiones: escaso número de enfermos remitidos para valoración rehabilitadora, pero con una elevada eficacia del tratamiento. Necesidad de establecer protocolos de valoración conjunta entre la unidad de Cuidados Paliativos y la unidad de rehabilitación. El papel de la rehabilitación dentro de la medicina paliativa está por desarrollar y nos encontramos todavía muy lejos de alcanzar el nivel deseado de integración entre ambas especialidades.

  Results: 1,172 interconsultations, sent from the Paliative Care Unit, were revised, and 33 (2.81%) were selected among them. Of the 33 patients, 23 started rehabilitation (69.7%). The main rehabilitation diagnoses were motor alterations and functional impotence as a sequela of muscular atrophy due to a long stay in bed. The most employed treatment technique was kinesitherapy. The average number of carried out rehabilitation sesions was 6. Therapeutic objectives were achieved in 60.9% of the cases.

  Conclusions: although very few patientes were sent for rehabilitation evaluation, the effectiveness of the treatment was high. Establishing connected valuation protocols is necessary. The role of rehabilitation within paliative medicine is still to be developed, and we are far from reaching the desirable level of integration between both specialities.
