Declaraciones anticipadas de tratamiento y asistencia de enfermería. Orientaciones decisionales de los enfermeros en la indagación empírica

    El presente trabajo constituye un análisis de una investigación empírica que ha tenido el objetivo de reflexionar sobre el tema de las declaraciones anticipadas de tratamiento, así como son perci- bidas en la asistencia de enfermería, buscando entender cuáles aspectos, en qué medida puedan influenciar la asistencia de en- fermería y cómo, en consecuencia, ésta podría evolucionar. Han sido explorados además algunos aspectos referentes a la forma- ción de los enfermeros sobre tal temática, las orientaciones deci- sionales de enfermería frente a estos documentos, y los aspectos deontológicos y profesionales que podrían modificarse en el futuro.

    De 64 enfermeros contactados, 32 han aceptado tomar parte en el estudio. A éstos ha sido suministrado un cuestionario subdi- vidido en modo de indagar los principales aspectos de la investi- gación. De la indagación empírica emerge la favorable adhesión de los enfermeros respecto de las declaraciones anticipadas de tratamiento, que son vistas como un documento que puede ser de ayuda tanto para el paciente como para el operador sanitario en momentos frágiles y delicados de la asistencia humana.

    De los resultados emerge también que para mejorar el signifi- cado de las declaraciones anticipadas será necesaria una ade- cuada formación de los enfermeros, sobre todo en ámbitos de la ética clínica y médico-legal, ámbitos en los cuales hoy parecen no estar preparados. Se pone en evidencia además, la importancia de la colaboración entre los miembros de un equipo para llegar a realizar una planificación asistencial compartida por los pacien- tes, o al menos definir mejor el posible rol que precisamente el enfermero podría asumir en el curso de la redacción de las decla- raciones anticipadas por parte del paciente, como resultado de la evolución de la figura del enfermero también como educador.

    This article proposes a description of an empirical research that has the aim to reflect on advance directives as perceived in nur- sing care, trying to figure out, in what ways and to what extent, they may affect nursing care and how they consequently will evol- ve. The study also investigated other aspects such as: the training of nurses on this theme, the perceptions of nurses about these documents and, finally, the aspect related to professional ethics that may change in the future.

    Of the 64 nurses contacted, 32 agreed to take part in the stu- dy. The questionnaire was elaborated to investigate the main areas of research. From the answers given, it emerges a favora- ble adhesion of nurses to advance directives, in so far these do- cuments are perceived as helpful for both the patient and the health care worker in a fragile and delicate moments of human existence.

    The results show, furthermore, that for improving the significance of advance directives, an adequate training of nurses is necessary, especially in the areas of clinical ethics and forensic medicine, where there is a paucity of information.
