Community-hospital coordination must be an objective shared by all health care professionals involved in the treatment of cancer. It requires a certain level of collaboration between all protagonists, i.e. the patient, the caregivers and the professionals, from diagnosis through to the post-cancer period. It must consider patients as a whole, meaning their physical condition, before, during and after the disease, their mental state and their socio-economic situation.

La coordination ville-hôpital doit être un objectif partagé par l’ensemble des acteurs de santé impliqués dans la prise en charge du cancer. Elle passe par un certain niveau de collaboration entre les protagonistes, à savoir le patient, les aidants et les professionnels, du dépistage à l’après-cancer. Elle doit tenir compte du patient dans sa globalité, c’est-à-dire de son état physique, avant, pendant et après la maladie, de son contexte psychique et de sa situation socio-économique.
