Biloma gigante tras hepatectomía en paciente con cáncer de colon metastásico

    Los bilomas son colecciones intraabdominales que ocurren tras resecciones hepáticas mayores aproximadamente en un 3% de los pacientes y son debidas a una fístula biliar provocada de forma yatrógena durante la disección de los radicales biliares. Se suelen manifestar por dolor abdominal y masa palpable y el diagnóstico fundamental es mediante pruebas de imagen. Su principal complicación es la sobreinfección que cursará con fiebre elevada, dolor abdominal y elevación de reactantes de fase aguda y el tratamiento en estos casos será antibioterapia intravenosa y drenaje de la colección. Presentamos el caso de un paciente con cáncer de colon y metástasis hepática que fue sometido a hepatectomía parcial y que, en el postoperatorio diferido, desarrolló un biloma hepático gigante que precisó tratamiento con drenaje percutáneo.

    Bilomas are intra-abdominal collections that occur after major liver resections in about 3% of patients and are due to a bile fistula caused by an estrogen during dissection of the bile radicals. They are usually manifested by abdominal pain and palpable mass and the fundamental diagnosis is by imaging tests. Its main complication is the superinfection that will occur with high fever, abdominal pain and elevation of acute phase reactants and the treatment in these cases will be intravenous antibiotic therapy and drainage of the collection. We present the case of a patient with colon cancer and liver metastases who underwent partial hepatectomy and who developed a giant hepatic biloma in the postoperative period that required percutaneous drainage treatment.
