Introducción y objetivos: El déficit de la enzima Diamino Oxidasa (DAO), principal enzima encargada del metabolismo de la histamina alimentaria, está sirviendo como nuevo diagnóstico para justificar la aparición de diferentes síntomas clínicos, como la migraña, la urticaria o algunos desordenes digestivos, entre otros. Todos ellos podrían relacionarse con la ingesta de alimentos ricos en histamina. El principal objetivo en ambos casos fue evaluar la eficacia de una dieta baja en histamina en dos pacientes con migraña y déficit de DAO.Material y métodos: Los dos casos estudiados tratan del seguimiento de una mujer de 39 años y de una mujer de 49 años. Ambas diagnosticadas de migraña que no mejoraban siguiendo el tratamiento farmacológico habitual, prescrito por el médico. Por consiguiente, se les analizó la actividad DAO después de darse cuenta de que el déficit de DAO podría relacionarse con otros síntomas que presentaban. Debido a los resultados obtenidos, fue necesario seguir un tratamiento dietético con una dieta baja en histamina, saludable y suplementada con enzima DAO exógena.Resultados: Después de algunas semanas siguiendo el tratamiento, el primer caso empezó a notar mejoría de los síntomas neurológicos. Después de meses de seguimiento, no solamente ha mejorado la migraña y la cefalea, sino también los síntomas respiratorios, digestivos y fatiga. Por otro lado, en el segundo caso, parece haber una pequeña mejora, pero los episodios de migraña continúan casi igual que al inicio.Conclusiones: Una dieta baja en histamina suplementada con enzima DAO debería considerarse como nuevo tratamiento en pacientes con migraña y déficit de DAO.
Introduction & aims: Deficiency of Diamine Oxidase (DAO) enzyme, the main enzyme responsible for histamine-food metabolism, is being used as a new diagnosis to justify the appearance of different clinical symptoms, such as migraine, urticaria or some digestive disorders, among others. All of them could be related to the intake of histamine-rich food. The aim in both cases was to assess the efficacy of a lowhistamine diet in two patients with migraine and DAO deficiency.Material and methods: The two case study involves the follow-up of a 39-year-old woman and a 49- yearold woman. Both of them diagnosed with migraine that did not improve following the usual medical treatment prescribed by the doctor. Therefore, DAO activity was analysed after realizing that DAO deficiency could be related to other symptoms that they presented. Due to the results obtained, it was necessary to follow a dietary treatment with a low-histamine healthy diet and exogenous DAO enzyme supplementation.Results: After some weeks following the dietary treatment, the first case began to notice improvement in neurological symptoms. After months of follow-up she has improved not only the migraine and headache, but also respiratory, digestive and fatigue symptoms. On the other hand, in the second case, it seems to be a little improvement but migraine episodes continue almost as in the beginning.Conclusions: A low-histamine diet supplemented with exogenous DAO enzyme should be considerate as a new management in patients with migraine and DAO deficiency.
