    • Terminal chronic renal failure is a syndrome that establishes itself slowly and progressively and which gives rise to an irreversible deterioration of kidney functioning. This irreversible deterioration of kidney functioning and the annihilation of the detoxification, endocrinological and metabolic missions of the kidney, which are eventually fatal, make it necessary to resort to haemodialysis. The fundamental aim of this process is the regulation of the water and electrolyte balance in the patient’s blood and the removal of the toxic waste it contains. It must be borne in mind that the loss of the endocrine and metabolic functions is not corrected with this therapy and pharmacological treatment accompanied by dietary recommendations will also be required. Of the numerous pathologies that affect different organic systems due to the appearance of chronic renal insufficiency, we have focused our research work on the study of iron metabolism and on the causes and effects of iron deficiency anaemia in patients receiving haemodialysis. We suggest that it can be corrected by the intravenous administration of an iron preparation. For reasons that only have to do with the legal regulations governing the National Health System, while following up our patients we had to change the pharmacological presentation of the iron supplement given to those on haemodialysis. This fact allowed us to study the behaviour of the two molecules and to compare, retrospectively, the results that were obtained. Following up our patients for a year after the application of this legal regulation enabled us to conduct a prospective study of how their condition varied without modifying other types of factors that could affect the results and the evaluation of the effects exerted on iron metabolism and erythropoiesis by treatment with the abovementioned new presentation of iron (IV). In this study we present the methodology that was used, the analysis of the results, their clinical and statistical meaning, and the conclusions that can be drawn from them. We must point out that, in our case, it is still early to establish a strictly economic evaluation of the preparation and the cost-effectiveness it could offer compared to another or in its joint evaluation with Rhu-EPO. Nevertheless, we must highlight the fact that its application in patients in predialysis in our service allowed us to conclude (although only empirically for the time being due to the short series of patients who were treated) that the results obtained in this type of patients are encouraging.
