Objetivo: Comparar la evolución clínica e histológica en dos modelos de tratamiento postoperatorio tras causticación corneal.
Material y métodos: Se practicó una causticación corneal con un filtro de 7.5 mm impregnado con una solución de NaOH 1N en 20 conejos pigmentados. En el postoperatorio los animales se dividieron: Grupo 1: Lente terapéutica y asociación de AINE (diclofenaco sódico) y grupo 2: lente terapéutica. Se valora diariamente la evolución clínica (neovasos corneales, vascularización conjuntival, defecto epitelial, leucoma) hasta el día 7. Se realiza un estudio histológico en el día 7 postratamiento.
Resultados: Al 4º día se inician los neovasos corneales afectando a 3 y 4 cuadrantes en el día 7. Tras la adaptación de la lente terapéutica la reepitelización se completaba en el día 5 para el grupo 1. No habían diferencias significativas del leucoma final para ambos grupos (57,37 ± 9,5 y 51,52 ± 8,4 mm).
Conclusiones: La lente terapéutica es eficaz en la fase aguda de la causticación corneal aunque se debe retirar al quinto día por facilitar la isquemia y neovascularización periférica.
Purpose: The aim of this study is compare the efficacy in acute chemical injury with two types of postoperative treatment.
Methods: Corneas of 20 pigmented rabbits were burned with 1 N NaOH by means of a filter paper 7.5 mm diameter, 30 sec exposure. The animals were divided into two groups of postoperative treatment: group 1: bandage contact lens in association with diclofenac sodium 0.1 % and group 2: only bandage contact lens. The animals were evaluated daily for 7 days. The following points were noted: corneal and conjunctival vascularization, corneal opacity area and epithelial defecto An histopathology study of the eyes was performed 7 days after.
Results: Corneal vascularization appeared on the 4th day with a higher degree on day 7 (3, 4).
Complete epithelial healing was established after 5 days in group 1. There was no significant difference in the final corneal opacity area. The histologic study revealed an increase of stratum epithelium and caliciform cells.
Conclusions: Use of a bandage contact lens with a topical nonsteroidal anti-inflammatory may prove to be an effective adjunct in treating traumatic corneal abrasions in the acute phase (in the first 5 days).
