    El sistema inmunológico es el conjunto de estructuras y procesos que le permiten al organismo el mantenimiento de la homeostasis ante cualquier factor agresor. Está altamente capacitado para evitar el desarrollo de la autoinmunidad. En los últimos años, ha cobrado importancia el concepto de los «puntos de control inmunitario», vías inhibitorias que regulan las respuestas inmunes en los tejidos periféricos. La vía del receptor de muerte celular programada (PD-1) representa uno de estos puntos de control y su función principal es el desarrollo de la autotolerancia. Algunas células tumorales son capaces de sobreexpresar dicho receptor y sus ligandos, siendo reconocidas por el sistema inmune como células propias del organismo, con lo que evitan su ataque y destrucción. Los objetivos de esta revisión serán definir la vía del receptor PD-1 y sus ligandos, conocer su rol como punto de control inmunitario y la importancia de su expresión aberrante en los microambientes tumorales, su papel en el desarrollo de inmunoterapia contra el melanoma metastásico, así como los medicamentos desarrollados hasta la actualidad como bloqueantes de dicha vía.

    The immune system is a group of structures and processes that allow the organism to maintain homeostasis and protect it against disease. It is highly trained to prevent the development of autoimmunity. In the past few years, the concept of « immune checkpoints » has become important. They are inhibitory pathways that regulate the immune responses in peripheral tissues. The programmed cell death receptor (PD-1) pathway represents one of these checkpoints and its main function is the development of self-tolerance. Some tumor cells are capable of overexpressing this receptor and its ligands, being recognized by the immune system as cells of the organism, thus preventing its attack and destruction. The objectives of this review will be to define the pathway of the PD-1 receptor and its ligands, to highlight its role as an immune checkpoint and the importance of its aberrant expression in the tumor microenvironment, its role in the development of immunotherapy against metastatic melanoma and to provide information on the drugs developed as blockers of this pathway
