    La Hipertensión Arterial Resistente (HTAR) afecta a un 3 – 29% de los hipertensos, segúnse defina el cuadro. En esta revisión analizamos los factores etiopatogénicos de HTAR, lasmedidas diagnósticas a seguir y las modificaciones terapéuticas más útiles. Se describen yanalizan la falta de adherencia a la medicación o a la dieta, la sobrecarga de volumen, loserrores de la medida de la presión arterial (PA), así como las indicaciones de registroambulatorio de 24h, o de los estudios de hipertensión secundaria, para concluir en unasrecomendaciones estratégicas resumidas.

    Resistant Arterial Hypertension affects to 3 - 29% of the hypertensive patients, dependingof the used definition. In this revision we analyzed the ethiopathogenic factors of resistanthypertension, the most useful diagnostic measures and the modifications of theantihypertensive therapy. We describe and analyze the lack of compliance to themedication or the diet, the volume overload, the errors of the measurement of the BP, aswell as the indications of 24 hr ambulatory registry, or the studies of secondaryhypertension, to conclude in a few summarized strategic recommendations.

    Hipertentsio Arterial Erresistentea (HTAR) hipertentsiboen %3-29ri eragiten dio,koadroa nola definitzen den. Errebisio honetan HTAR faktore etiopatogenikoakaztertuko ditigu, jarraitu beharreko neurri diagnostikoak eta aldaketa terapeutikoerabilgarrienak. Medikazioari edo dietari zaion atxikidurarik eza deskribatu etaaztertuko da, baita bolumen gainkarga, PAren neurketaren errakuntzak, 24 ordukoerregistro anbulatorioko jarraibideak eta bigarren mailako hipertentsio azterlanak ere,gomendio estrategiko laburbildu batzuekin bukatzeko.
