The recent discoveries on the genetic and molecular background of hypercortisolism have radically improved the knowledge of the pathophysiology of Cushing’s syndrome and promoted the research for promising pharmacologic compounds that might change treatment strategies in the coming years. Moreover, the increasing popularity of disease-specific registries across and within Europe led to the creation of large retrospective databases with reliable data even on a rare condition like Cushing’s syn- drome. Therefore, the long-term data on the natural history of Cushing’s syndrome allowed a critical revision of the clinical practice on the management of this condition.
