Cómo prevenir y tratar las lesiones de mancha blanca durante y después del tratamiento de ortodoncia fija

    Introducción: El tratamiento de ortodoncia con aparatología fija es un factor de riesgo de desmineralización del esmalte. Cerca de un 50% de los pacientes desarrollan al menos una caries en estadio inicial: lesiones de mancha blanca o white spot lesion (WSL) durante el tratamiento. La magnitud del problema y el hecho de que cada vez más personas se plantean un tratamiento ortodóncico genera la necesidad de conocer y evaluar los mecanismos de prevención y tratamiento más efectivos y eficaces a nivel mundial. Material y métodos: Se realizó una revisión bibliográfica en PubMed, Cochrane, Embase y Scopus hasta Mayo de 2017. Resultados y discusión: Como elementos protectores se observa el flujo salivar correcto y la terapia preventiva con flúor durante el tratamiento. En la prevención y el tratamiento de las lesiones de mancha blanca aparecen tratamientos que no necesitan del apoyo y motivación del paciente porque se realizan en el gabinete dental, mientras que otros tratamientos deben ser realizados por los pacientes en su casa. Como métodos preventivos podemos aplicar selladores de fisuras, resina de flúor de alta concentración, CCP-ACP o CCP-ACFP con resultados positivos. Como métodos de tratamiento para reducir la diferencia estética con el resto del esmalte del diente o dientes adyacentes se emplean el blanqueamiento, la microabrasión y la microinfiltración, con diferentes resultados, y la aplicación de flúor y CCP-ACP o CCP-ACFP con diferentes resultados. Conclusión: No aplicar ninguna medida preventiva durante el tratamiento de ortodoncia, dejando la enfermedad a su evolución natural, no es éticamente correcto. Por tanto, la prevención o, en el peor de los casos, el tratamiento de la aparición de las lesiones de mancha blanca ha de ser una de las metas de la ortodoncia moderna.

    Introduction: The orthodontic treatment with fixed appliances is a risk factor for the enamel demineralization. Nearly a 50% of patients develop, at least, a cavity in the initial phase: white spot lesion (WSL), during the treatment. The magnitude of the problem, and the fact that more and more people are planning to do orthodontic treatment, creates de necessity to acknowledge and evaluate more effective mechanisms of prevention and treatment. Material and methods: A bibliographic research is done in Pub-Med, Cochrane, EMBASE and SCOPUS until May 2017. Results and discussion: During treatment a proper saliva flux and the use of fluoride acts as preventing therapy. Some treatments for treating and preventing the white spot lesions do not need the patient’ support or motivation because are done at the dental office, and others need the compliance of the patients at home. As a preventing methods sealers, high concentrated fluoride resin, or the usage of CCP-ACP or CCP-ACFP can be used, all of them with positive results. As treatment methods to reduce the aesthetical difference with the rest of the teeth enamel or nearby tooth, dental bleaching, micro abrasion and micro infiltration and the application of Fluoride and CCP-ACP or CCP-ACFP can be used, with different results. Conclusion: Not applying any preventing measure during the orthodontic treatment, and leaving the disease at its own natural evolution, is not ethically correct. Therefore, during the prevention phase, or in the worst of the cases, the treatment of white spot lesions, must be one of the goals of modern orthodontics.
