Tratamiento médico del cáncer colorrectal metastásico a partir de la 3ª línea

    En los últimos años, dos nuevos agentes orales se han incorporado al arsenal terapéutico en los pacientes con cáncer colorrectal metastásico refractario (CCRm): el inhibidor multiquinasa Regorafenib y el citotóxico trifluridina/tipiracilo.

    Ambos tratamientos han demostrado mejorías significativas en supervivencia global (SG) en pacientes con CCRm. Sus diferentes perfiles de toxicidad son importantes a la hora de seleccionar la mejor terapia para cada paciente. Repasaremos sus distintos mecanismos de acción, la eficacia reportada en los diferentes estudios realizados hasta la fecha, su toxicidad y las diferentes estrategias para optimizar su uso y manejar los principales efectos adversos. Plantearemos posibles opciones de secuenciación en la 3ª línea, basadas en las características clínicas de cada paciente, sus comorbilidades y efectos adversos residuales de tratamientos previos. Finalmente, resumiremos la escasa evidencia científica hasta la fecha en relación a la eficacia de la reintroducción de líneas previas. El futuro en el tratamiento de estos pacientes resulta esperanzador aunque aún hay un largo camino por recorrer. Diferentes estudios en marcha definirán el rol de cada terapia, así como de la combinación con otros agentes.

    In recent years, two new oral agents have been incorporated to the therapeutic arsenal for patients with refractory metastatic colorectal cancer (mCRC), the multitargeted tyrosine kinase inhibitor regorafenib and the cytotoxic trifluridine/ tipiracil. Both treatments have demonstrated significant improvements in overall survival (OS) in patients with refractory mCRC. Their different side effect profiles are important in the selection of the best therapy for each patient. We will review their action mechanisms, the efficacy reported in the different clinical trials performed to date, their toxicity profile, the different strategies to optimize their use and the management of their main adverse events. We will discuss different options of treatment sequence in 3rd line, based on patient characteristics, such as comorbidities and adverse reactions to previous treatments. Finally, we will do a brief overview of the little scientific evidence regarding the reintroduction of previous therapies in the 3rd line. Future is looking brighter for these patients, although we still have a long way to go. Ongoing studies will further define the role of these agents and their role in the combination with other treatments.
