Cirugías de reemplazo articular en pacientes jóvenes

    En el pasado muchos especialistas eran reacios a indicar el reemplazo articular en pacientes jóvenes por temor a condenarlos a múltiples revisiones durante el resto de su vida. Con la tecnología actual, las prótesis de reemplazo, cuya selección se ajusta a las demandas funcionales, asociadas a la edad del paciente, constituyen una alternativa para pacientes jóvenes y activos.Objetivo: reflexionar sobre el comportamiento actual de la cirugía de reemplazo articular en el paciente joven.Desarrollo: Se delimitó el material informativo, se estudiaron los patrones semánticos y de citación a fin de determinar las principales líneas de investigación desarrolladas en el mundo para el estudio del tema, fueron aplicados criterios de selectividad que favorecieron, al equipo de investigación, centrarse en los documentos relevantes. Se procedió a organizar de manera sistemática la documentación encontrada, posteriormente se desarrolló el análisis cualitativo de la información en los grupos temáticos más significativos. Una vez identificados los documentos principales, se llevó a cabo un análisis de co-citación de autores y de co-ocurrencia. Finalmente, desde el punto de vista cuantitativo, se resumieron aspectos como número de documentos por año, documentos, citaciones por autor, documentos e investigaciones realizadas por país, entre otros. Teniéndose que de los 66 documentos iniciales, 40 fueron identificados como de mayor interés. Estos, a su vez, fueron filtrados y leídos con mayor detalle, seleccionándose 35 de verdadera relevancia para la investigación. Luego de ejecutarse la lectura detallada, finalmente se descartaron 2 artículos y se eligieron 31, un libro y una tesis de maestría, a partir de los cuales se elaboró el estado del arte sobre el tema.Conclusiones: Aunque la artritis suele afectar a los pacientes mayores, en las últimas décadas el comportamiento actual de la cirugía de reemplazo articular en el paciente joven ha sufrido transformaciones. Existe una población de pacientes "jóvenes"en rápido crecimiento que experimentan síntomas debilitantes de la enfermedad. Las expectativas de estos pacientes más jóvenes y los desafíos para optimizar atención de salud, son a menudo soslayados. Sin embargo la misma, en ocasiones involucra una cirugía de reemplazo. Esta se asocia a un conjunto de elementos que van desde la selección del implante, de la superficie de apoyo y de la técnica quirúrgica que pueden afectar el éxito y la longevidad del reemplazo.

    In the past many specialists were reluctant to indicate joint replacement in young patients for fear of condemning them to multiple revisions for the rest of their lives. With current technology, replacement prostheses, whose selection adjusts to functional demands associated with the patient's age, are an alternative for young and active patients.

    Objective: to reflect on the current behavior of joint replacement surgery in the young patient.

    Development: information material was delimited, semantic and citation patterns were studied in order to determine the main lines of research developed in the world for the study of the subject, selectivity criteria were applied which favored the research team to focus on the relevant documents. The documentation was systematically organized, and the qualitative analysis of the information was then developed in the most significant thematic groups. Once the main documents were identified, an analysis of co-citation of authors and co-occurrence was carried out. Finally, from a quantitative point of view, we summarized aspects such as number of documents per year, documents, citations by author, documents and research carried out by country, among others. Given that of the 66 initial documents, 40 were identified as of greater interest. These, in turn, were filtered and read in greater detail, selecting 35 of real relevance for the investigation. After completing the detailed reading, 2 items were finally discarded and 31 papers, a book and a master's thesis were chosen, from which the state of the art on the subject was elaborated.

    Conclusions: Although arthritis usually affects older patients, in recent decades the current behavior of joint replacement surgery in the young patient has undergone transformations. There is a rapidly growing population of "young" patients experiencing debilitating symptoms of the disease. The expectations of these younger patients and the challenges to optimize health care are often overlooked. However, it sometimes involves replacement surgery. This is associated with a set of elements ranging from the selection of the implant, the surface of support and the surgical technique that can affect the success and longevity of the replacement.
