Lactancia materna y mastitis: Tratamiento empírico basado en la sintomatología y los agentes etiológicos

    Las mastitis representan la principal causa médica de destete precoz. Generalmente, las mastitis agudas se diagnostican sin dificultad, pero no así las mastitis subagudas o las subclínicas.

    Para el tratamiento de los diferentes tipos de mastitis sería deseable disponer de los resultados de un cultivo y de un antibiograma. Sin embargo, actualmente son muy escasos los servicios de microbiología que realizan análisis microbiológicos de este tipo de muestras. En este contexto, el objetivo de este artículo era relacionar cada tipo de mastitis con los agentes etiológicos, la sintomatología característica y el tratamiento empírico más adecuado. La propuesta de tratamiento está fundamentalmente dirigida a los casos en que no sea posible la realización de cultivos y antibiogramas y/o en los que no se pueda esperar a los resultados para iniciar el tratamiento.

    Mastitis represents the main medical reason for precocious weaning. Generally, acute mastitis is easily diagnosed, in contrast to the subacute o subclinical ones. Milk cultures and antibiograms would facilitate a rational treatment of the different types of mastitis; however, the services of Microbiology that routinely perform microbiological analyses of human milk are very scarce at present. In this context, the objective of this article is to associate each type of mastitis with its etiological agents, its characteristic symptoms and the most adequate therapeutic approach. This therapeutic proposal is basically directed to those cases in which milk cultures and antibiograms are not possible and/or those in which is not possible to wait for the results in order to initiate the treatment.
