Heart failure is a syndrome that entails sudden appearance or worsening signs and symptoms of congestive heart failure and low cardiac output. It requires an urgent diagnosis and treatment. It can be the first episode or might appear as an acute decompensation of chronic heart failure. Patient assessement It is important to identify trigger factors that may also need urgent treatment. There are many classifications of acute heart failure but the most useful is the one based on clinical presentation, that not only gives prognostic information but creates a pathway for initial treatment. So it is classified according to the appearance of congestion (wet/dry) or hypoperfusion (cold/warm). Supplementary tests The diagnosis in not always easy and it must be confirmed by several tests: ECG, chest x-ray, echocardiography and laboratory tests that include natriuretic peptides.
