Nocardiosis and actinomycosis are chronic granulomatous infections caused by Gram-positive bacilli of the order Actinomycetales. These are characterized by branching filaments that can develop into mycelia. Both diseases are causes of chronic cavitary sepsis and are being seen increasingly in immunocompromised patients, in whom they can result in disseminated abscess formation, often resulting is misdiagnosis as other serious pathologies such as tuberculosis and invasive fungal infection. Traditionally in both conditions, relatively long courses of antibiotic therapy are required for resolution of disease. Recent advances in diagnostic techniques and increasing evidence for the use of a combined medical and surgical approach to the treatment of these infections, allowing shorter treatment courses; have led to a revolution in their management.
