    • Diabetes mellitus is a serious chronic condition with significant impact on morbidity and mortality. The prevalence of type 2 diabetes in particular continues to rise worldwide and is imposing a significant burden on health economies. Community and general practice services are ideally placed to provide much of the care in a diabetes care pathway, and to undertake prevention, screening, surveillance and routine management in line with evidence-based practice. Appropriately trained community healthcare professionals, with the support of diabetes specialists, can also provide more complex care effectively for many patients. In the UK, commissioners of services are now putting in place quality-assured pathways for diabetes care that aim to deliver high-quality care for their populations.
