Erectile dysfunction affects over a third of men with diabetes mellitus. It is caused by failure of nitric oxide-mediated smooth muscle relaxation of the corpus cavernosum secondary to endothelial dysfunction and autonomic neuropathy. Phosphodiesterase type 5 inhibitors are the first-line treatment and are effective in about 50–60% of men with diabetes. Alternative treatments include vacuum devices and self-injection treatment.
