Le réseau Paris Diabète est une association qui coordonne le suivi pluridisciplinaire des patients diabétiques parisiens. En recueillant l’adhésion de professionnels de santé, il leur offre un espace d’échanges et de réflexion commun. Les patients adhérents bénéficient d’un accès facilité à un parcours de soins adapté et modulable au sein du réseau. Un dispositif d’éducation thérapeutique est mis en place à leur intention, animé par les professionnels du réseau, et des outils, pédagogiques et de suivi, sont à la disposition de tous. Au sein de ce dispositif, le pharmacien d’officine adhérent peut assurer un suivi efficace et régulier des patients diabétiques.

The Paris Diabetes care network is an association coordinating the multidisciplinary follow-up of patients with diabetes in Paris. By including the membership of health professionals, it provides an area for common reflection and discussion. Patient members enjoy easy access to a course of tailored and scalable treatment within the network. A therapeutic educational device led by network professionals has been set up for their benefit and educational and follow-up tools are available to all. Within this system, the pharmacist member can provide effective and regular monitoring of diabetic patients.
